REQUIRED DOCUMENTS
On the day of the interview, each applicant, regardless of visa class, must submit the following documentation:
- Two bank receipts to prove that the non-refundable application fee was paid at a branch of Armeconombank.
- A completed and signed Nonimmigrant Visa Application Form (DS-156) with a photo attached. This form is available at http://evisaforms.state.gov. Please note that each electronic form should include a barcode used for scanning by consular personnel. Therefore, applications with poor-quality barcodes (i.e. poor printer quality, broken line barcode) will not be accepted. The visa applicants will be requested to resubmit their applications.

- A Supplemental Nonimmigrant Visa Application, Form DS-157 provides additional information about your travel plans. Submission of this completed form is required for all male applicants between 16-45 years of age. It is also required for all applicants from state sponsors of terrorism age 16 and over, irrespective of gender, without exception. Five countries are now designated as state sponsors of terrorism, including North Korea, Cuba, Syria, Sudan, and Iran. - Students and exchange visitors (F1/M1/J1) must present a completed form DS-158 in addition to the DS-156 and DS-157 (if applicable). This form is available at http://travel.state.gov/visa/frvi/forms/forms_1342.html.
- A passport valid for travel to the United States and with a validity date at least six months beyond the applicant's intended period of stay in the United States. Armenian citizens must also obtain an exit stamp from the Armenian state passport agency valid for at least three months beyond the intended travel date.
- One recent 2"x2" (5x5 cm) photograph for each applicant glued to the DS-156 form. The photograph must not be more than six months old. The photograph must be a front view of the applicant´s entire face with a plain white background. Sunglasses or other articles which detract from the face are not acceptable unless required for medical reasons (an eye patch, for example). The applicant´s head must not be covered.
